Initial Enrollment Period | IEP | You can join a Medicare Advantage Plan or Medicare Supplement and Drug Plan | Seven-month period, this includes the three months before, the month of, and the three months following your 65th birthday. |
Initial Coverage Election Period | ICEP | One can join a Medicare Advantage (MA) only Plan | This is used when an individuals has different Part A and B effective dates. Use this election period 3 months prior to the Part B effective date. |
MA OEP | OEP | One can change Medicare Advantage plans, drop their Medicare Advantage plan and return to Original Medicare and join a Prescription Drug Plan (PDP). | This is a one-time election from January 1st – March 31st. |
Move | MOV | One can change a MA, MAPD, or PDP When they change addresses | They will have the month before, the month of and 2 months after. |
Five Star Plan | 5ST | One can change or join a Medicare Advantage Plan. | This can only be used once from December 8th – November 30th. |
Dual Eligible, LIS Eligible | MDE | This allows an individual to enroll in, or disenroll from, a Medicare Advantage (MA) plan once per calendar quarter during the first nine months of the year. | Begins the month the individual becomes dually eligible and exists as long as he or she receives Medicaid benefits. Can be used once during each of the following time periods: January – March April – June July – September |
Lose, Gain , or Have a Change in Dual Eligible Status | MCD | This allows an individual to enroll in, or disenroll from, a Medicare Advantage plan (Ma) or a Prescription Drug Plan (PDP). | Allows the individual one opportunity to make an election within three months of any of the changes noted or notification of such a change, whichever is later. |

Federal State or local disaster | DST | This allows an individual to enroll in, or disenroll from, a Medicare Advantage plan (Ma) or a Prescription Drug Plan (PDP). | Used for individuals affected by a disaster or other emergency which must be declared by a Federal, state, or local government entity. Must have been unable to and did not make an election during another valid election period. SEP starts as of the date the declaration and ends two full calendars months following the end date identified in the declaration. |
Other Credible Coverage | OCC | Enroll into a Medicare Advantage or Medicare Advantage Prescription Drug PLan MAPD | This allows individuals enrolled in a PDP or MAPD may disenroll to enroll or maintain other creditable drug (such as Tricare, VA or Champ VA coverage) by using this SEP to enroll in a MA or MAPD plan. Can be used once per Calendar year. |
Lost Employer Coverage | LEC | Enroll into a Medicare Advantage (MA) , Medicare Advantage with Prescription Drug plan (MAPD or Prescription Drug Plan (PDP) | This is used for individuals who are losing group health coverage (including COBRA). They will have the month they lose coverage plus two months after their coverage has ended. |
Release from Incarceration | INC | One can enroll into a MA, MAPD or PDP | This begins the date of release from incarceration or with the date the individual provides notification of such move and continues for two full calendar months. |
Lost Credible Coverage | LCC | On can enroll into a Medicare Advantage Prescription Drug Plan (MAPD) or Prescription Drug Plan (PDP) | Usen when there is an involuntary loss of credible Prescription Drug coverage or a reduction in the level of coverage so that it’s no longer credible. They will have the month they lose coverage plus two months after the coverage has ended. |

Loss of Special Need Status | SNP | One can enroll into a Medicare Advantage(MA), or Medicare Advantage with Prescription drug plan (MAPD) or Prescription Drug plan (PDP) | For individuals enrolled in a SNP who are no longer eligible for the SNP because they no longer meet the specific special needs status. They can make changes the month they lose eligibility or are notified of the loss, whichever is earlier, and ends two months after either the month of the loss of eligibility or notification of the loss, whichever is later. |
Chronic Condition | CHR | One enroll into a C-(Special Needs Plan (SNP) or disenrolling and enrolling into a Medicare Advantage (MA), or Medicare Advantage with Prescription Drug plan (MAPD), or (Prescription Drug Plan)PDP | Used for Individuals with a chronic condition. This can be used once per calendar year.Used for individuals who are not found or no longer have a chronic condition necessary to enroll into this plan. Begins when they are notified and have two additional months.Individual is enrolled in a C-SNP who has a chronic condition which is not the focus of their current C-SNP. |
Moved back to the U.S. | RUS | One cannroll into a Medicare Advantage, or Medicare Advantage Prescription Drug Plan or Prescription Drug Plan. | Begins the date of the move or with the date the individual provides notification of such move and continues for two calendar months. |
State Pharmaceutical Assistance Program | PAP | One can enroll into a Medicare Advantage (MA), or Medicare Advantage with Prescription drug Plan (MAPD) or Prescription Drug plan ( PDP) | Individuals who belong to a SPAP are eligible to make one enrollment request per year into another Ma, MAPD or PDP. Additionally, individuals who disenroll from PACE have a SEP for up to two calendar months after the effective date of the PACE disenrollment. |
Trial Right 1 | 12J | Enroll into a PDP | Individuals who elected a MA/MAPD plan during their IEP/ICEP surrounding their 65th birthday have this SEP allows them to disenroll from the MA/MAPD and return to Original Medicare (with or without a PDP) anytime during the 12-month period that begins on the effective date of coverage in the MA/MAPD plan. |
Trial Right 2 | 12G | One can enroll into a Presription Drug Plan (PDP) | This SEP is for individuals who dropped a Medicare Supplement (Medigap) plan after enrolling in an MA/MAPD for the first time, and who are still within 12 calendar months of that first MA/MAPD enrollment. Can elect to disenroll from the MA/MAPD, return to Original Medicare (with or without a PDP). They will also have guaranteed issue to return to the Medicare Supplement they had before (same Supplement and same company) |
